1. Brief introduction to the company (2 originals)
It should include corporate application, date of registration, address, name of board chair or general manager, registered capital, business scope, credit standing and respectability, branches and trading condition with China.

2. Letter of Application (2 originals)
It should be written to Sichuan Provincial Administration for Industry and Commerce, signed by board chair or general manager and stamped with corporate seal. The corporate operation condition, date of registration, registered capital, trading or investing condition in China, reason of setting representative office in Sichuan and its name, address, the head (chief representative), time limit and business scope should be specified.

3. Letter of Nomination (2 originals)
It should be a document written to Sichuan Provincial Administration for Industry and Commerce, signed by board chair or general manager, for chief representative nomination and representative nomination. (If board chair or general manager appoints himself to be the chief representative, the letter of nomination should be signed by chairman of board and more than 2 directors at one time.)

4. Letter of authorization (2 originals)
The letter of authorization is signed by board chair or general manager, and used to authorize chief representative to sign on the relating documents of applying representative office.

5. Certification of credit standing and respectability (1 original)
It should be written by the bank of deposit of head office. And the digit of deposit, date of account opening, and credit standing should be specified in it.

6. Official business license of foreign (outside borders) legal investor (1 original)
The copy of license, which is offered, should be notarized by the notary organ in the state (area) of head office. And an original of certification of the local consulate of China is available. (As for the organization or enterprise of Hong Kong, a copy of effective business certification of registration in this year should be offered as well.)

7. Resume of chief representative and representatives (1 copy)
It should begin with the date of work beginning or with higher education. It should be detailed and continuous.

8. ID of chief representative
As for the person from foreign countries, Hong Kong, Macao and Taiwan, a copy of passport, travel certificate or reentry permit should be offered; while a Chinese citizen should offer a copy of ID card.

9. 2-inch pictures of chief representative and representatives (5 pictures)

10. One personal seal of chief representative(used for industrial and commercial registration, and opening an account, etc.)

11. Certificate for office building use of representative office (leasing agreement or house purchasing contract) (1 copy)
In industrial and commercial registration, the original of house purchasing or renting contract will be checked.

Remarks: Chinese translation of all materials, which must be stamped with the official seal of the translation company (or CCPIT SICHUAN), should be provided at the same time.
